Fill out the following form with the name and address where you want the product shipped. Be sure to input a quantity for the item or items that you want (if no quantity is entered, we can not fill your order).Then press the submit button. Then print out or copy the page which comes up and send it with your check or money order in the correct amount to the address stated on the form. DO NOT put credit card information in this form. We can only accept checks or money orders at this time.
